Professional License Search
It is not necessary to fill in all the fields to do the search, you can put in just the last name, or last name and city, or just the license number, or last name and Board (please check below for proper three letter code for search) to be able to pull up the information needed for your search.
BOARD:
MED – Physician and Paramedic
PAD – Physician Assistant
If you want details on the licensee, click on the license number in blue.
Under “Additional Information” if no information is provided, that means there are no disciplinary actions taken on that licensee.
