Durable Medical Equipment Vendors

The Alaska Board of Pharmacy requires registration of a pharmacy located outside of the state ONLY if the pharmacy ships, mails or delivers prescription drugs to consumers in the state (AS 08.80.158).

If your facility offers only medical equipment and medical devices, you do not need to be registered with the board of pharmacy because you are not providing prescription drugs (medications).

Other application forms may be mailed to you or picked up at a division office. Completed applications must be signed and submitted to the division with the appropriate fees.
